IMPORTANT: Neurology Associates requires a written referral from your physician prior to scheduling an appointment.

Please arrive early (15-30 minutes) to complete paperwork and insurance information. Please understand that the time of your new patient experience will last approximately one hour which includes any wait time, evaluation and treatment plan formulation. Medicine is a highly variable discipline and can require extra time due to complex cases and unforeseen emergencies. Your patience is greatly appreciated.

Please bring the following to your appointment:
  • Insurance card(s)
  • Picture ID such as a driver's license.
  • Referral form if required by your insurance provider
  • Any recent test results or appropriate records including MRI(s), CT scan(s) and any lab results.
  • Completed patient forms OR a complete list of all prescription and over-the-counter medications you are currently taking (including dose and frequency) and pertinent information about your medical and surgical history.
  • Method of Payment- We accept cash, check, debit cards, and Visa & Mastercard.
  • If your an auto accident patient please bring your claim number and your medical insurance card, if any.
